How?
Step 1: You can access the necessary forms via this link. Open “otoplastics form”.
Step 2: Complete the form and email it to Freddy Josemanders ([email protected])
Step 3: After registration and completion of the request, the person making the request will receive an email with his or her form signed.
Step 4: Go with the signed form to a hearing care professional:
- Make an appointment with a hearing care professional, preferably where you live
(Adresses via http://www.intersafe.eu/nl/servicepunten) - The hearing care professional will make an imprint of the auditory canal and sends this together with the order voucher to Intersafe-Groeneveld
- Intersafe-Groeneveld produces the otoplastics and sends them to the hearing care professional.
- The hearing care professional rings the employee when the otoplastics are ready and on collection gives a short instruction on how to use them.
NB: Always make an appointment with the hearing care professional beforehand!
